LenSen Forte: Each tablet contains: Ibuprofen 400 mg, Paracetamol 325 mg.
Pharmacology: Pharmacodynamics: Mechanism of Action: Paracetamol inhibits the synthesis of prostaglandins in the central nervous system and peripherally blocks pain impulse generation; produces antipyresis from inhibition of hypothalamic heat-regulating center.
Ibuprofen inhibits prostaglandin synthesis by decreasing the activity of the enzyme, cyclooxygenase, which results in decreased formation of prostaglandin precursors.
Pharmacokinetics: Paracetamol is readily absorbed from the gastrointestinal tract with peak plasma concentrations occurring about 10 to 60 minutes after oral doses. It crosses the placenta and is present in breast milk. Plasma-protein binding is negligible at usual therapeutic concentrations but increases with increasing concentrations. The elimination half-life of paracetamol varies from about 1 to 3 hours.
Ibuprofen is absorbed from the gastrointestinal tract and peak plasma concentrations occur about 1 to 2 hours after ingestion. Ibuprofen is 90 to 99% bound to plasma proteins and has a plasma half-life of about 2 hours. It is rapidly excreted in the urine mainly as metabolites and their conjugates. About 1% is excreted in urine as unchanged ibuprofen and about 14% as conjugated ibuprofen.
LenSen Extra: Relief of mild to moderate pain of musculoskeletal origin e.g. low back syndrome, bursitis, arthritis, rheumatism, myalgias, sprains, strains & traumatic injuries.
LenSen Forte: LenSen Forte is an analgesic. It provides symptomatic relief from pain in the following conditions:
Soft tissue non-articular rheumatism eg. myositis, fibrositis, tenosynovitis.
Traumatic injuries due to accidents, sports injuries, sprains, strains, fractures and dislocations, contusions.
Infective inflammation due to tonsillitis, abscess, pleurisy; to be used along with appropriate chemotherapy.
Acute exacerbation of rheumatoid or other similar inflammatory arthropathies, where pain is the distressing symptom.
LenSen Extra: 1-2 tablets 3 times daily immediately after meal.
LenSen Forte: 1 tablet 6 to 8 hourly after a meal, do not exceed 6 tablets per day.
Should not be used in presence of peptic ulcer, kidney or bone marrow impairment; hypersensitivity to the drug, asthma and in pregnancy.
Should be used with caution in patients with a history of impaired function of eye, liver, kidney, heart or bone marrow.
LenSen Extra/LenSen Forte may cause GI upsets or gastritis. Even though LenSen Extra/LenSen Forte belongs to the family of NSAIDs, it causes side effects associated with this class of drugs like hepatitis, nephropathy or bone marrow depression. Rarely, blurred vision, dizziness, oedema or rashes have been reported. There may be cross allergenicity with other NSAIDs.
Paracetamol: Increased Effect/Toxicity: Barbiturates, carbamazepine, hydantoins, isoniazid, rifampin, sulfinpyrazone may increase the hepatotoxic potential of acetaminophen; chronic ethanol abuse increases risk for acetaminophen toxicity; effect of warfarin may be enhanced.
Decreased Effect: Barbiturates, carbamazepine, hydantoins, rifampin, sulfinpyrazone may decrease the analgesic effect of acetaminophen; cholestyramine may decrease acetaminophen absorption (separate dosing by at least 1 hour).
Ibuprofen: Increased Effect/Toxicity: Ibuprofen may increase cyclosporine, digoxin, lithium, and methotrexate serum concentrations. The renal adverse effects of ACE inhibitors may be potentiated by NSAIDs. Corticosteroids may increase the risk of GI ulceration.
Decreased Effect: Aspirin may decrease ibuprofen serum concentrations. Ibuprofen may decrease the effect of some antihypertensive agents (including ACE inhibitors and angiotensin antagonists) and diuretics.
